A few years ago both rear seat belts failed and were replaced, costing around NZ$1000. Now one of these belts has also failed . It won't retract. I rarely carry passengers so the belt has had minimal use. This was the case with the initial failed belts. Will this keep happening? What to do?

Weird I have never heard of this happening, I would first check if there are any recalls and if there aren't any then you would have to bring it to dealer or a local shop.
